Purpose

This policy explains how California Prime Recovery ensures continuity of care across admission, treatment, transitions between levels of care, discharge, and post-discharge follow-up to support positive clinical outcomes.

Our Policy

California Prime Recovery provides structured, coordinated care across all programs and services. Continuity of care is maintained during both planned and unplanned transitions to prevent gaps in treatment, promote stability, and support ongoing engagement in recommended services.

Scope of Care

This policy applies to all California Prime Recovery programs, including:

Continuity of Care in Treatment

During Admission and Treatment

Continuity of care begins at admission. Individualized treatment plans identify clinical needs, referrals, transitions between levels of care, and aftercare planning as treatment progresses.

Transitions Between Levels of Care

When a client transitions between programs or services, transition planning is initiated early to ensure coordination, communication, and appropriate follow-up. This includes internal transitions and referrals to external providers when needed.

External Referrals

When services outside California Prime Recovery are recommended, referrals are made with client consent and shared in accordance with confidentiality requirements. Follow-up efforts support continued engagement in care.

Discharge and Post-Discharge Support

Planned Discharge

Discharge planning begins during treatment. Clients receive a discharge summary, aftercare recommendations, and referral information to support continued recovery.

Unplanned Discharge or Leaving Against Clinical Advice

If a client leaves treatment early, reasonable efforts are made to encourage continued care, provide referrals, and support safety. Continuity of care efforts continue whenever possible.

Post-Discharge Follow-Up

Post-discharge follow-up calls are conducted to assess client well-being, encourage engagement with recommended services, and identify the need for additional support or re-engagement in care.

Alumni and Aftercare

When available, alumni or aftercare opportunities may be offered to support ongoing connection and recovery. Participation is voluntary and does not replace clinical or community-based care.

Readmission

Clients seeking readmission are evaluated according to admission criteria. Prior treatment history is reviewed to support continuity and appropriateness of care.

Client Rights

Clients are active participants in care planning and transitions. Services are provided in accordance with client rights, informed consent, and confidentiality laws. Decisions to decline services or referrals are respected and documented.

Our Commitment to Coordinated Care

California Prime Recovery believes continuity of care is essential to safety, stability, and long-term recovery. Through coordinated planning, follow-up, and collaboration, we support each client’s recovery journey beyond a single level of care.
